We're hiring a Hospice Case Manager (RN) who wants meaningful one-on-one patient relationships, schedule stability, and the opportunity to provide compassionate end-of-life care while maintaining an excellent work-life balance.

You'll step into a hospice field role providing direct patient care in homes and care facilities. You'll manage your own patient caseload while collaborating with an experienced interdisciplinary team dedicated to delivering exceptional hospice services. You'll practice throughout the Warwick, Rhode Island area, a coastal community offering beautiful waterfronts, historic neighborhoods, and convenient access to Providence.

What makes you a great fit
• You hold an active Rhode Island RN license or a Compact RN license eligible for Rhode Island
• You have at least one year of nursing experience
• Hospice or home health experience is strongly preferred

Compensation & Benefits
• Base salary: $104,000–$114,400 annually
• Health & dental insurance: Employer pays 60% of the employee premium
• Vision insurance

• Sick time: 40 hours annually
