Position Summary
The Case Manager coordinates and organizes hospice services for assigned patients to ensure seamless care delivery and regulatory compliance. This role focuses on care coordination, scheduling, documentation, and quality assurance.
Key Responsibilities
Services & Care Coordination
● Coordinate ongoing patient care, services, and scheduling among interdisciplinary team members.
● Handle orders management (DME, medications, supplies) and confirm timely fulfillment.
● Track issues and outcomes, and document changes.
IDG (Interdisciplinary Group) Support
● Prepare for IDG notes and submit required updates.
● Assist with recertifications by gathering required documentation and coordinating with the RN & MD.
Discharge & Transitions
● Coordinate live discharges with the team and community partners.
● Coordinate the death pronouncements per licensure/scope and agency protocol.
● Support discharge by death processes (documentation, communication, family support-bereavement).
● Ensure HIS (Hospice Item Set) discharge data accurately and ready for submission.
Quality Assurance
● Monitor clinical, and discharge documentation for accuracy and timeliness.
● Follow up on missing or incomplete notes and obtain corrections from staff.
● Ensure compliance with agency and Medicare standards at all times.
