We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Northbrook, IL area and is eligible for a $5,000 Sign-on Bonus.
What You’ll Do:
- Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
- Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
- Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
- Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
- Work independently, but never alone
- Travel to patient homes within assigned territory

CNA Qualifications
- Active Certified Nursing Assistant (CNA) certificate in the state you work
- High School Diploma or certificate
- Minimum one year healthcare experience, Hospice experience preferred
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
