Hospice Certified Nursing Assistant

About the Role:
Solace Hospice & Palliative Care Inc is looking for a compassionate and dedicated Hospice Certified Nursing Assistant to join our caring team in Saint Charles, IL. In this meaningful role, you'll provide hands-on personal care and emotional support to patients and families during life's most important moments. Responsibilities:

• Provide personal care services including bathing, grooming, oral hygiene, and repositioning for hospice patients
• Assist with activities of daily living (ADLs) in patient homes or care facilities
• Monitor and report changes in patient condition, vital signs, and comfort levels to the nursing team
• Offer compassionate emotional support and companionship to patients and their families
• Assist with light housekeeping, meal preparation, and personal laundry related to patient care
• Document patient care activities accurately and in a timely manner
• Collaborate with the interdisciplinary hospice care team to ensure a holistic patient experience

Requirements:

• Current CNA certification in the state of Illinois
• Prior hospice, palliative care, or end-of-life care experience preferred
• Strong understanding of comfort-focused, patient-centered care principles
• Excellent communication and interpersonal skills with patients, families, and care teams
• Reliable transportation and valid driver's license for home visit travel in the Saint Charles, IL area
• Ability to work independently with compassion, patience, and professionalism
• CPR certification and ability to meet health screening requirements
