As a Hospice Client Care Coordinator (CCC), you will be called to care when you’re needed most. As part of Interim HealthCare, you’ll support a full range of patient services to bring comfort and dignity to our clients.

As a Hospice Client Care Coordinator (CCC), you will:
- Facilitate new client enrollment with initial and subsequent screening assessments and further maintain client’s healthcare record
- Identify resources to address client’s health, daily living, and safety needs
- Perform administrative tasks such as scheduling, logging, documenting all appointments and services
- Communicate to clients through telephone and computer
- Coordinate with RN and pharmacy medication set-ups and reminders
- Be a family liaison and coordinate with the family, designees, and providers
To qualify to be a Hospice Client Care Coordinator (CCC) with us, you will need:
- Education: High School graduate or equivalent
- One year of related healthcare experience is required
- Experience in working in the senior field or hospice is preferred
- Must have a valid driver’s license
