What You’ll Do as a Hospice C.N.A:
+ Deliver personal, compassionate care in alignment with the patient’s care plan
+ Support patients and families in learning self-care techniques
+ Provide comfort, dignity, and safety during end-of-life care
+ Communicate important updates to the RN Case Manager and care team
+ Document patient conditions, medication updates, and care provided
+ Take steps to reduce avoidable hospital visits and ensure continuity of care
+ Serve as a trusted, positive presence for patients, families, and colleagues
What We’re Looking For:
+ High School Diploma or GED
+ At least 1 year of experience as a Hospice Aide or Nursing Assistant
+ Completion of a state-approved Hospice Aide training program with competency evaluation (per 42 CFR §484.36)
+ Home health or hospice experience preferred
+ Reliable transportation, valid driver’s license, and auto insurance (50% travel required)
+ Ability to lift 50–100 lbs and perform physical caregiving duties
