Job Title:Hospice CNA | The Heart of Our Care Team

Compensation:$19-$25/hour DOE + mileage reimbursement paid by wage + $500 Sign-On Bonus

Benefits: Full benefits package available for full-time staff

Schedule: Per Diem or Full-Time | Weekend rotation

Location: 21-County Twin Cities Service Area

What You'll Do

• Assist patients with bathing, dressing, grooming, toileting, and mobility.
  
• Monitor patient comfort and report changes in condition to the nursing team.
  
• Provide companionship and emotional support to patients and families.
  
• Help create a safe, clean, and comfortable environment.
  
• Follow individualized care plans while working closely with nurses and the interdisciplinary team.
  
• Deliver care that promotes dignity, comfort, and quality of life.
  

Who We're Looking For

• Current Minnesota CNA certification.
  
• Hospice experience preferred, with home care, assisted living, long-term care, hospital, or oncology experience also considered.
  
• Compassionate, observant, and dependable.
  
• Excellent communication and collaboration skills.
  
• Valid driver's license and reliable transportation.
