Responsibilities:- Travel between multiple patient locations throughout your shift (no single 1:1 assignments)
- Provide personal care and support services to patients
- Assist with daily living activities, including bathing, dressing, mobility support
- Offer comfort and compassion to hospice patients and their families during end-of-life care
- Monitor, report, and document patient conditions
- Collaborate with the healthcare team
- Maintain accurate patient visit records
Requirements:- Current CNA certification
- Must have at least 1 year of experience
- Valid driver's license and insured vehicle
- Previous home health and hospice experience preferred
- Experience working with geriatric patients
- Compassionate, patient-focused attitude
