Job Summary
Harmony Home Health & Hospice is seeking a Hospice Community Liaison to join our growing hospice team. The ideal candidate will serve as an ambassador for our hospice program by building trusted relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and community partners while educating patients and families about the benefits of hospice care.

In addition to external business development, this role will play an integral part in Harmony'sPathways Program, working collaboratively with our home health clinicians to identify patients who may benefit from goals-of-care conversations and hospice education. The Hospice Community Liaison serves as both an advocate and educator, ensuring patients and families have the information needed to make informed decisions aligned with their goals and values. 

This position is Full Time in the Salt Lake Valley  

Responsibilities 
  • Develop and maintain referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and community organizations.
  • Increase awareness of Harmony Hospice services through regular provider visits and educational presentations. 
  • Represent Harmony at networking events, health fairs, and community outreach opportunities.  
  • Identify opportunities to grow hospice admissions while maintaining the highest ethical standards.
Qualifications
  • Experience in hospice, palliative care, home health, and healthcare sales.
  • Excellent communication and relationship-building skills. 
  • Ability to educate clinicians, physicians, patients, and families. 
  • Valid driver's license and reliable transportation.
