As the Hospice Home Team Leader, you'll play a vital role in ensuring our inpatient hospice team delivers outstanding clinical care, compassionate support, and an exceptional experience for every patient and family entrusted to us.

This role is ideal for an experienced hospice nurse who enjoys mentoring staff, solving complex operational challenges, and leading by example.
What You'll Do
Lead Clinical Operations
- Supervise, prioritize, and coordinate patient care activities during assigned shifts within the Hospice Home
- Ensure continuity of care and outstanding patient outcomes
- Monitor staffing needs and assign admissions appropriately
- Coordinate care delivery among nursing staff and hospice aides
- Support interdisciplinary collaboration to achieve patient and family goals
Develop and Support Clinical Staff
- Participate in interviewing, hiring, onboarding, and orientation of new clinical team members
- Mentor, coach, and support nurses and hospice aides
- Foster a positive and collaborative team culture
- Identify staff development opportunities and competency needs
- Partner with education and quality teams to ensure clinical excellence
Ensure Quality and Regulatory Compliance
- Conduct regular chart audits to ensure documentation accuracy and compliance
- Monitor compliance with Medicare, Medicaid, ACHC, state, and organizational requirements
- Review and implement clinical policies, procedures, and best practices
- Promote quality improvement initiatives and operational excellence
- Ensure accurate and timely clinical documentation
Support Patient and Family Care
- Partner with patients, families, physicians, and interdisciplinary team members to address care concerns and clinical issues
- Assist with complex care coordination and problem-solving
- Ensure compassionate, patient-centered care remains at the forefront of all decisions
Manage Operational Responsibilities
- Oversee medication room operations and regulatory compliance
- Ensure safe management of narcotics and medications
- Support scheduling and staffing needs
- Conduct performance evaluations and personnel management activities
- Participate in Management Backup (MBU) coverage as assigned
- Cover at least one holiday annually

Required Qualifications
- Current Registered Nurse (RN) license in North Carolina
- Minimum five years of recent clinical experience in hospice, home health, or related healthcare settings
- Current CPR certification
- Thorough understanding of hospice clinical interventions and standards of practice
- Knowledge of Medicare, Medicaid, regulatory, and accreditation requirements
- Strong leadership, communication, delegation, and organizational skills
- Computer proficiency and ability to learn new software systems
- Current unrestricted North Carolina driver's license
- Reliable transportation
- Ability to lift up to 30 pounds
Preferred Qualifications
- Previous supervisory or leadership experience
- Experience managing interdisciplinary teams
- Hospice inpatient unit experience
- Experience with performance management, staff development, and coaching
- Knowledge of clinical quality improvement processes
- Experience with healthcare documentation and electronic medical records
