Hospice Home Health Aide (HHA)

Overview

We are seeking a compassionate Home Health Aide (HHA) to provide care and support to hospice patients throughout Tompkins and Cortland Counties.

Schedule

• Full-Time
• Monday-Friday
• 8:30 AM-4:30 PM
• 1-hour lunch break

Responsibilities

• Follow the patient's plan of care as directed by the nurse.
• Report changes in patient condition to the appropriate nurse.
• Document care provided accurately and timely.
• Assist patients with prescribed exercises.
• Provide personal care, bathing, and hygiene assistance.
• Prepare meals and assist with feeding when needed.
• Perform light housekeeping to maintain a safe environment.
• Make beds and help maintain patient comfort.
• Provide respite support for family caregivers.
• Attend required in-services and team meetings.
• Maintain a safe environment and report concerns promptly.
• Perform other duties as assigned.

Requirements

• Current New York Home Health Aide (HHA) Certification required.
• Valid driver's license.
• Reliable transportation.
• Ability to travel throughout Tompkins and Cortland Counties.
