Overview
We are seeking a compassionate Registered Nurse (RN) for a full-time Hospice Case Manager position. This role involves providing skilled nursing care to hospice patients in their homes while working independently in the field as part of a supportive interdisciplinary team.

Requirements
- Active New York Registered Nurse (RN) License
- At least 2 years of RN patient care experience
- Valid driver's license and reliable transportation
- Comfortable working independently in the field
- Authorized to work in the United States. No employer-sponsored work visas available
