What You’ll Do
  • Identify the patient’s and family’s physical, psychosocial, environmental, safety, and developmental needs and individualize the plan of care.
  • Document nursing assessments, problems, goals, interventions, and outcomes achieved.
  • Assess all aspects of pain and develop an individualized pain management plan.
  • Anticipate, prevent, and treat undesirable symptoms and secondary symptoms.
  • Participate in interdisciplinary team (IDT) meetings to establish, review, and revise the plan of care.
  • Maintain up-to-date patient records to ensure accuracy, completeness, and compliance with applicable standards.
  • Teach patients, caregivers, and appropriate support personnel about interventions, medications, effects, and side effects.
  • Coordinate patient and family services while supporting comfort-focused, dignified end-of-life care.

What You Need Required
  • RN license
  • Hospice experience
  • HCHB experience
  • Strong documentation and electronic medical record (EMR) accuracy/compliance skills
