What Youll Do
- Identify the patients and familys physical, psychosocial, environmental, safety, and developmental needs and individualize the plan of care.
- Document nursing assessments, problems, goals, interventions, and outcomes achieved.
- Assess all aspects of pain and develop an individualized pain management plan.
- Anticipate, prevent, and treat undesirable symptoms and secondary symptoms.
- Participate in interdisciplinary team (IDT) meetings to establish, review, and revise the plan of care.
- Maintain up-to-date patient records to ensure accuracy, completeness, and compliance with applicable standards.
- Teach patients, caregivers, and appropriate support personnel about interventions, medications, effects, and side effects.
- Coordinate patient and family services while supporting comfort-focused, dignified end-of-life care.
What You Need
Required
- RN license
- Hospice experience
- HCHB experience
- Strong documentation and electronic medical record (EMR) accuracy/compliance skills
