What Youll Do:
- Manage hospice cases with patient-centered care plans
- Assess physical, psychosocial, environmental, safety, and developmental needs
- Document assessments, interventions, and patient outcomes accurately
- Develop and implement pain management strategies
- Monitor and address symptoms, including comfort and end-of-life needs
- Collaborate with interdisciplinary team meetings to review and revise care plans
- Educate patients, families, and caregivers on medications, treatments, and coping strategies
- Maintain comprehensive and compliant medical records
- Support emotional, spiritual, and cultural needs of patients and families
- Ensure dignity and holistic support for dying patients
- Coordinate services across care settings in accordance with national standards
What You Need:
- Valid RN license in Texas
- Hospice care experience required
- Familiarity with HCHB electronic health records
- Strong communication and compassionate care skills
- Ability to collaborate within interdisciplinary teams
