Hospice RN Case Manager – Fort Worth, TX | Day Shift | $50.25/hr

Pay & Benefits:

What You’ll Do:
  • Manage hospice cases with patient-centered care plans
  • Assess physical, psychosocial, environmental, safety, and developmental needs
  • Document assessments, interventions, and patient outcomes accurately
  • Develop and implement pain management strategies
  • Monitor and address symptoms, including comfort and end-of-life needs
  • Collaborate with interdisciplinary team meetings to review and revise care plans
  • Educate patients, families, and caregivers on medications, treatments, and coping strategies
  • Maintain comprehensive and compliant medical records
  • Support emotional, spiritual, and cultural needs of patients and families
  • Ensure dignity and holistic support for dying patients
  • Coordinate services across care settings in accordance with national standards

What You Need:
  • Valid RN license in Texas
  • Hospice care experience required
  • Familiarity with HCHB electronic health records
  • Strong communication and compassionate care skills
  • Ability to collaborate within interdisciplinary teams
