Job Description Summary

The Inpatient Hospice Hospital RN serves within Corewell Health West Michigan hospitals, providing hospice consultations, eligibility assessments, care coordination, General Inpatient (GIP) hospice evaluations, and discharge planning support for patients with serious illness and end-of-life care needs. The role collaborates closely with physicians, care managers, social workers, bedside nurses, patients, and families to facilitate timely transitions to hospice services and ensure care aligns with patient goals and preferences.

Responsibilities include conducting hospice informational visits, coordinating admissions to GIP hospice care, facilitating transitions to home hospice, nursing facilities, assisted living communities, or hospice inpatient settings, and supporting hospital throughput through timely hospice interventions. The position requires advanced communication skills, hospice expertise, symptom management knowledge, and the ability to navigate complex clinical and psychosocial situations in a fast-paced hospital environment.

Key Responsibilities

Assess patients for hospice eligibility and appropriate level of care.

Conduct hospice informational visits and goals-of-care discussions with patients and families.

Coordinate GIP hospice admissions and symptom-management interventions.

Facilitate timely transitions to home hospice, Hanson Hospice Center, SNF, or ALF settings.

Collaborate with hospital providers and interdisciplinary teams to support discharge planning and continuity of care.

Serve as a hospice resource to hospital staff regarding hospice philosophy, eligibility, and end-of-life care.

Ensure accurate and timely documentation to support regulatory and quality standards.

Contribute to organizational goals related to hospice access, referral conversion, patient experience, and hospital throughput.

Qualifications

Required

• Associate's Degree Nursing
• 1 year of relevant experience experience, preferably in hospice or related field
• Registered Nurse (RN) - State of Michigan And
• BLS American Heart Association 90 Days

Preferred

• Bachelor's Degree in Nursing
• Experience with dying patients and their families
