Be the Best Hospice Inpatient Visit You Can Be
If you meet these qualifications, we would love to meet you:
- Maintains an active license and DEA in the state where care is being provided.
- Willing to make hospital inpatient visits one week per month (Tuesday - Monday schedule)
Job Requirements:
- Develop and maintain Plan of Care for each inpatient, including discharge planning discussions with care team, patients, and families
- Participate in prognosis evaluation and certification/recertification
- Sign death certificates as needed
- Conduct daily patient rounds and set consistent rounding schedules with interdisciplinary team
- Ensure proper documentation of eligibility for inpatient care
- Provide education and training to interdisciplinary team and new staff
- Provide after-hours call coverage on days with inpatient visits
