    Description

    University Home Care Inc is seeking a compassionate and organized Hospice Coordinator to join our team in Livonia, MI. The Hospice Coordinator will play a critical role in providing support and coordination for patients receiving hospice care, ensuring a smooth and compassionate experience for patients and their families.

    Responsibilities

    • Coordinate hospice care services between patients, families, healthcare providers, and community resources.
    • Develop and maintain comprehensive care plans tailored to each patient’s needs.
    • Serve as a primary contact for patients and families to address questions, concerns, and provide emotional support.
    • Collaborate with interdisciplinary teams to promote the highest quality end-of-life care.
    • Arrange and schedule visits by hospice care professionals and community service providers.
    • Document and maintain accurate records in compliance with company policies and regulatory requirements.
    • Participate in staff meetings and training sessions to stay updated on hospice care best practices.

    Requirements

    • Prior experience in hospice care coordination  at least a year or related healthcare field preferred.
    • Strong organizational and communication skills.
    • Ability to provide compassionate support to patients and families.
    • Knowledge of hospice care regulations, procedures, and documentation standards.
    • Proficient with electronic medical records (EMR) and Microsoft Office Suite.
    • High school diploma or equivalent required; additional healthcare-related certification is a plus.
