This is an in-office position located in Santa Rosa

Schedule: 8am-4:30pm PST

Position Summary

An Intake assessment Nurse LVN is a LVN with home health experience whose primary responsibility is to accept referrals from a variety of resources via phone and fax.

Position Specific Responsibilities

  • • Prioritizes daily referrals in order to guarantee timely patient admission.
  • Answers telephone calls promptly and courteously.
  • Checks and follows up on ECIN and iNEXX referrals on a daily basis.
  • Scans referral information into All Docs.
  • Accurately transcribes medical and insurance information from faxed documentation and phone referrals.
  • Verifies and obtains orders from physician.
  • Ascertains if insurance is contracted with SJHH in order to assure appropriate financial reimbursement.
  • Interfaces with the Insurance Verification Specialist as needed.
  • Communicates with Clinical Manager and Service Coordinator to assure coordination of services.
  • Keeps Pharmacy Department informed of upcoming referrals and faxes them the final hard copies.
  • Applies Medicare and State Regulations pertaining to home health when accepting a referral in order to be compliant with said regulations.
  • Create and maintain intake orientation manual and assess caregiver skills to identify training needs.
  • Instruct and monitor the work of orientees.
  • Provide comprehensive and constructive feedback for intake team members and update Director of Nursing.
  • Makes recommendations related to training and skill development to the Director of Nursing.
  • Create and manage on-call schedules of RNs and LVNs for weekdays, weekends and holidays.
  • Coordinate referrals at intake with QA, eligibility ensuring initial authorization for visits and corrections needed for authorizations, e.g., med records, diagnosis etc.
  • Assist with resolution of referral source complaints, concerns or requests.
  • Review referral packets to ensure completion.
  • Provide daily declination reports to the Executive Director.
  • Contact referral sources to provide availability of disciplines and geographical locations of available resources.
  • Create and manage schedules for intake staff.
  • Performs other duties as assigned.

Education and/or Experience

  • Required - Coursework/Training Completes necessary education to attain LVN Licensure in state of California.
  • Required - 1 year Acute care experience.
  • Required - 1 year Home health experience.

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Language Skills: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
  • Computer Skills: Knowledge of computers.

  • Other Skills and Abilities: Knowledge of home health compliance issues, payer coverage requirements related to eligibility for services.
  • Other Skills and Abilities: Knowledge of home health referral process preferred.

Certifications, Licenses, and Registrations

  • Required upon hire: California Licensed Vocational Nurse (Vendor Managed)
  • Required upon request: Driving may be necessary as part of this role.
  • Caregivers are required to comply with all state laws and requirements for driving.
  • Caregivers will be expected to provide proof of driver license and auto insurance upon request.

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range: $34.90 - $53.39

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
