Hospice Liaison – Conroe Territory

We are partnering with a leading national hospice organization seeking a compassionate, relationship-driven Hospice Liaison to support growth across the Conroe territory. This is a rewarding opportunity for a healthcare sales professional who wants to combine strong earning potential with meaningful, patient-centered work.

In this role, you will serve as a key ambassador in the community—building trusted relationships with referral partners while helping patients and families navigate one of the most important transitions in care.


What You’ll Do

  • Develop and grow referral relationships with physicians, hospitals, SNFs, and other healthcare providers
  • Maintain and strengthen existing partnerships to drive consistent hospice admissions
  • Coordinate and manage the referral process, ensuring a seamless and timely patient experience
  • Serve as a trusted resource and educator for providers, patients, and families
  • Collaborate with clinical teams to ensure smooth patient transitions into hospice care
  • Execute territory growth strategies to meet and exceed census and revenue goals

What We’re Looking For

  • Proven success in medical sales, with strong preference for hospice or home health business development
  • Demonstrated ability to build and grow a referral network
  • Strong interpersonal skills with a compassionate, consultative approach
  • Highly organized with attention to detail in managing referrals and documentation
  • Comfortable working in a field-based role covering the Conroe territory
  • Proficiency with Microsoft Office and CRM tools
  • Ability to travel locally up to 50%
