Join Novant Health as a Hospice Liaison Nurse and serve as a vital connection between patients, families, healthcare providers, and hospice services. In this highly collaborative role, you'll help ensure timely access to hospice care by coordinating referrals, conducting clinical assessments, facilitating admissions, and supporting seamless transitions from acute care settings to hospice services.
The primary focus of this role is referral processing and admission scheduling, ensuring eligible patients receive the right care at the right time. You'll partner closely with physicians, case managers, discharge planners, social workers, and healthcare teams to provide education, advocacy, and support throughout the hospice transition process.

What We're Looking For:
Education: H4 Year / Bachelors Degree, preferred. Graduate of an accredited school of nursing required; BSN preferred.
Experience: Minimum three years' experience in home care or hospice, required. Previous experience in managed care, infusion care, and case management and previous home health/hospice case management experience, preferred.
Licensure/Certification: Current RN licensure in appropriate state, required.

Ability to provide direct patient care, demonstrate excellent nursing skills
Excellent verbal and written communication skills, problem solving skills, and basic math skills.
Understands hospice philosophy and issues of death/dying and demonstrates competency in pain and symptom management with EOL care.
Ability to interpret information to identify each patient’s requirements for care relative to his/her age specific needs.
Ability to prioritize, organize and handle varying and unpredictable situation such as emergencies or crisis situations.
Must be flexible, versatile, adaptable to change, with good critical thinking, problem solving and diplomacy skills.
Knowledge of procedures and nursing standards of practice, both general and specific to the specialties of medical- surgical nursing.
Ability to interact effectively with diverse individuals at all levels of authority, in teams and on committees.
Actively identifies new areas for learning; regularly creating and taking advantage of learning opportunities; using newly gained knowledge and skill on the job and learning through their application.
Must have computer experience and have reliable transportation to and from all work related assignments and activities.
Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair.
Must be available for after hours call on rotational basis.
Specific vision abilities required by this job include close vision, distance vision, night driving, and ability to adjust and focus.
What You'll Do:
Process incoming hospice referrals and coordinate admission scheduling
Conduct comprehensive clinical assessments to determine hospice eligibility and patient needs
Facilitate hospice admissions and ensure smooth transitions from hospitals and other healthcare settings
Collaborate with physicians, case managers, social workers, discharge planners, and interdisciplinary healthcare teams
Educate patients, families, and referral sources on hospice services, benefits, and eligibility requirements
Support discharge planning efforts and help remove barriers to timely hospice placement
Build and maintain strong relationships with hospital staff, providers, and community partners
Ensure compliance with hospice regulations, clinical standards, and organizational policies
Advocate for patients and families by promoting comfort, dignity, and quality of life
Coordinate care to ensure exceptional patient and family experiences throughout the transition process
Work Environment
Primarily hospital-based position serving as an on-site liaison between Novant Health hospitals and hospice services
Position is based at Huntersville Medical Center (HMC)
May be required to provide coverage at any of Novant Health's five affiliated hospitals based on business needs
Occasional field-based admissions and patient visits may be required
Schedule: Friday, Saturday, and Sunday | 8:00 AM - 8:00 PM
