    Job Requirements

    POSITION SUMMARY

    The Hospice Licensed Clinical Social Worker (LCSW) coordinating the psychosocial care and services for assigned patients in collaboration with the patient, family, physician, patient care team, and payers. The Hospice Licensed Clinical Social Worker utilizes advanced psychosocial skills to facilitate the coordination of care through assessment, planning, intervention and evaluation. The Hospice Licensed Clinical Social Worker promotes and evaluates the effective utilization of resources using current knowledge, awareness of community services, and serving as a role model to achieve optimal clinical and resource outcomes.

    PRIMARY ACCOUNTABILITES

    • Completes psychosocial assessment to screen for patients/families at risk for crisis, abuse, financial or environmental issues and collaborates with interdisciplinary team to identify needs.
    • Acts as a facilitator on complex cases, make Department of Children and Families (DCF) referrals for suspected safety, neglect, potential abuse, or complex family issues.
    • Recommends supportive and therapeutic interventions as indicated to ensure a safe and efficient plan of care and quality patient outcomes.
    • Identifies and documents necessary transitional care needs (Assisted Living Facility (ALF), Long-Term Care (LTC) facility, respite, and transfer to inpatient level of care).
    • Collaborates to determine desired goals and communicates all options to patient's, families, Licensed Practitioners and staff members.
    • Identifies "at risk" patient populations and strengths of patients and family support systems. Assess' safety planning to include suicide ideation and in crisis Baker Act.
    • Reinforces and documents education with patient/family on psychosocial aspects of care and expected outcomes.
    • Initiates appropriate referrals to Ethics Committee, Risk Management or Legal services as appropriate.
    • Participates in resolution of legal issues such as guardianship, power of attorney, etc.
    • Assists with the implementation of advanced directives.

    Work Experience

    MINIMUM QUALIFICATIONS

    • Education: Master's degree in Social Work (MSW).
    • Work Experience: Three (3) years of previous experience in Social Work.
    • Licensure:
    • Clinical Social Worker licensure in the State of Florida.
    • Valid Driver's License and clean Motor Vehicle Report (MVR) and maintained.
    • Certification: American Heart Association (AHA) CPR AED HeartSaver course prior to start date and maintained.
    • Background Check: Must satisfy Florida Agency for Health Care Administration (ACHA) Level two (II) background screening.
    • Skills/Knowledge/Abilities:
    • Maintains current knowledge and awareness of payer and reimbursement practices, and regulations impacting the patient''s plan of care.
    • Demonstrates professional accountability through supporting patient''s rights, informed consent and advanced directives.

    PREFFERED QUALIFICATIONS

    • Work Experience: Five (5) years of previous experience in Social Work in an Acute Care, Hospice, or Home Care setting.
    • Skills/Knowledge/Abilities: Proficient in Microsoft Office - Outlook, Word, Excel, PowerPoint.

    PHYSICAL REQUIREMENTS

    • Physically agile; may require walking, standing, running, bending, stooping, kneeling, climbing, or crouching frequently.
    • May occasionally include lifting or moving objects up to 50 pounds, with or without assistance.
    • May require moving in different positions to accomplish tasks in various environments including tight and confined spaces, including reaching out and above shoulders, or overhead.
    • May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to 90% of the assigned shift.
    • May be exposed to inside and outside environments with varied temperatures, air quality, lighting, and/or low to loud noise.
    • May require working with biohazards such as blood/body fluids and airborne particles following OSHA and CDC guidelines.
    • Communicating with others to exchange information.
    • Visual acuity and hand-eye coordination to perform tasks.
    • Workspace may vary from open to confined.
    • May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
