At Cornerstone VNA, our mission is to provide trusted, compassionate, and expert care where it matters most - at home. We're seeking a dedicated Hospice Licensed Nursing Assistant (LNA/CNA) to join our caring and collaborative hospice team in the Portsmouth area.
As a Hospice LNA, you'll provide essential personal care and comfort to patients and their families, helping to promote dignity, independence, and quality of life during every stage of care.
What You'll Do
Apply the hospice philosophy of care to all patients and families.
Assist patients with activities of daily living (ADLs/IADLs) and personal care in accordance with individualized care plans.
Support patients with eating, mobility, repositioning, and comfort measures.
Take and record vital signs, collect specimens, and carry out basic treatments as directed by nursing staff.
Collaborate with hospice nurses, interdisciplinary team members, and families to ensure coordinated, compassionate care.
Document care and observations accurately in the EMR.
Maintain a safe, clean, and supportive environment for patients and families.

What You Bring
Current NH Licensed Nursing Assistant (LNA) license required
Maine CNA license preferred (required if seeing patients in Maine)
Minimum of 1 year of experience as an LNA/CNA preferred
Completion of required health and physical exams
Access to an insured, reliable vehicle
Comfortable using technology and mobile documentation tools
