Hospice Licensed Practical Nurse (LPN)
Position Overview
We are seeking a compassionate and dedicated Hospice Licensed Practical Nurse (LPN) to join our team in New Berlin, WI. The ideal candidate will provide high-quality nursing care to patients in hospice, ensuring comfort and dignity during end-of-life care. This role requires strong clinical skills, empathy, and the ability to work collaboratively with interdisciplinary teams. This position requires on-call responsibilities also.
Key Responsibilities
- Provide skilled nursing care to hospice patients in accordance with established care plans and physician orders.
- Monitor patient condition and report changes to the interdisciplinary team promptly.
- Administer medications and treatments as prescribed, ensuring patient safety and comfort.
- Assist with pain management and symptom control to enhance patient quality of life.
- Educate patients and families on disease progression, care techniques, and available resources.
- Document nursing care and patient responses accurately and timely in compliance with regulatory standards.
- Collaborate with physicians, social workers, chaplains, and other team members to develop and update individualized care plans.
- Provide emotional support to patients and their families during the hospice journey.
Qualifications
- Current Licensed Practical Nurse (LPN) license in the state of Wisconsin.
- Previous experience in hospice or palliative care preferred.
- Strong knowledge of hospice nursing principles and end-of-life care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate, patient-centered approach to care.
- Basic computer skills for documentation and communication.
Work Environment
This position involves providing care in patients' homes, nursing facilities, or hospice centers within the New Berlin area. The role requires flexibility, empathy, and the ability to manage emotionally challenging situations with professionalism and sensitivity.
