We are seeking a compassionate and dependable Hospice LPN to join our interdisciplinary team. This role is ideal for a nurse who values relationship-based care and wants to make a meaningful difference in patients’ lives.
✨ What You’ll Do
Under the supervision of a Registered Nurse, the Hospice LPN provides skilled nursing care to patients in home and facility settings in accordance with the individualized Plan of Care.
Clinical Responsibilities
- Provide skilled nursing visits as assigned
- Perform focused assessments and report findings to RN Case Manager
- Administer medications and treatments as ordered
- Monitor pain and symptom management effectiveness
- Provide wound care per physician orders
- Reinforce patient and family education on disease process and care plan
- Assist with coordination of care alongside the interdisciplinary team
- Participate in IDG meetings as appropriate
- Complete timely and accurate documentation in EMR
✅ Qualifications
- Active, unrestricted Virginia LPN license
- Hospice or home health experience preferred
- Strong assessment and communication skills
- Knowledge of end-of-life care principles
- Current CPR certification
- Valid driver’s license and reliable transportation
- Ability to travel throughout service area (mileage reimbursed)
