Hospice Medical Director

About the Role:
Mi Casa Healthcare LLC is seeking a compassionate and experienced Hospice Medical Director to join our dedicated team in San Antonio, TX. This is a meaningful opportunity to lead clinical excellence and provide expert guidance in end-of-life care, making a profound difference in the lives of patients and their families.



Responsibilities:

• Provide medical oversight and clinical leadership for the hospice program in accordance with federal and state regulations
• Collaborate with the interdisciplinary team (IDT) to develop and review individualized patient care plans
• Conduct and oversee patient eligibility certifications and recertifications for hospice services
• Serve as a clinical resource and consultant for attending physicians, nurses, and care team members
• Participate in quality assurance and performance improvement (QAPI) initiatives
• Ensure compliance with Medicare/Medicaid hospice conditions of participation
• Support staff education and training on palliative and end-of-life care best practices

Requirements:

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution
• Active and unrestricted Texas medical license
• Board certification in hospice and palliative medicine preferred
• Prior experience in hospice, palliative care, or a related clinical setting
• Strong knowledge of Medicare/Medicaid hospice regulations and compliance standards
• Excellent communication, leadership, and interdisciplinary collaboration skills
• Compassionate, patient-centered approach to end-of-life care
