Position Summary
Visiting Nurses Association (VNA) is seeking a compassionate and experienced PRN Medical Social Worker (MSW) to provide psychosocial assessments, counseling, and resource coordination to hospice patients and their families. This role focuses on supporting patients through end-of-life care by addressing emotional, social, and practical needs in accordance with physician orders, hospice plans of care, and regulatory requirements.
Key Responsibilities
- Conduct comprehensive psychosocial assessments for hospice patients and families
- Provide emotional support, counseling, and crisis intervention related to end-of-life care
- Assist patients and families with advance care planning, community resources, and bereavement support
- Collaborate with nurses, physicians, chaplains, and the interdisciplinary hospice team (IDT)
- Participate in care planning meetings and case conferences as required
- Ensure compliance with hospice, Medicare, state, and agency regulations
- Complete documentation accurately and within required timeframes
- Participate in required compliance training and departmental development activities
Qualifications
- Master's degree in Social Work (MSW) from an accredited program
- Current, unrestricted social work licensure in the State of Kansas (LSCSW, LMSW, or LCSW, as applicable)
- Minimum of one (1) year of hospice, home health, or healthcare experience preferred
- Knowledge of hospice philosophy, end-of-life care, and community resources
- Strong communication, documentation, and organizational skills
- Ability to work independently while collaborating with an interdisciplinary team
- Valid driver's license, reliable transportation, and auto insurance
