We’re looking for a compassionate PRN Medical Social Worker (MSW) to join our growing Phoenix hospice team. This is a great opportunity for an experienced social worker seeking a flexible schedule while making a meaningful difference in the lives of patients and families.

What You’ll Do

• Complete psychosocial assessments for hospice patients and their families.

• Develop individualized plans of care addressing emotional, social, financial, and resource needs.

• Provide counseling, crisis intervention, grief support, and emotional guidance.

• Assist patients and families with advance care planning and end-of-life decision-making.

• Connect patients and caregivers with appropriate community resources.

• Collaborate with nurses, physicians, chaplains, aides, and other members of the interdisciplinary hospice team.

• Maintain accurate and timely documentation in accordance with agency, state, and federal requirements.

• Participate in care conferences and interdisciplinary team meetings as needed.

Qualifications

• Master’s Degree in Social Work (MSW) from an accredited program.

• Previous hospice experience preferred.

• Strong communication, counseling, and interpersonal skills.

• Ability to work independently in the field while collaborating with the hospice team.

• Valid Arizona driver’s license, reliable transportation, and current auto insurance.
