Looking for a flexible Nurse Practitioner role where you can focus on meaningful patient care instead of rushed appointments and high-volume schedules?

At Concho Hearts Hospice, our Nurse Practitioners have the opportunity to build meaningful relationships with patients and families while providing compassionate end-of-life care. We are seeking a PRN Nurse Practitioner (NP) to support patients in the Beaumont, Port Arthur, and Orange areas.

What You’ll Do:
  • Complete face-to-face visits and clinical assessments for hospice patients.
  • Evaluate hospice eligibility and recertification requirements in collaboration with the Medical Director.
  • Provide symptom management and palliative support to patients and families.
  • Complete history & physical assessments and clinical documentation.
  • Collaborate with physicians, nurses, social workers, and the interdisciplinary hospice team.
  • Communicate clinical findings and recommendations to support quality patient care.

What We're Looking For:
  • Active Texas Nurse Practitioner license.
  • PECOS enrollment required for hospice certification requirements (or willingness to obtain).
  • Minimum 1 year of NP experience required.
  • Hospice, palliative care, family practice, home health, internal medicine, or related experience preferred.
  • Strong assessment, communication, and critical thinking skills.
  • Ability to work independently while collaborating with a team.
