We are seeking a PRN Nurse Practitioner (NP) to support patients in the Beaumont, Port Arthur, and Orange areas.
What You’ll Do:
- Complete face-to-face visits and clinical assessments for hospice patients.
- Evaluate hospice eligibility and recertification requirements in collaboration with the Medical Director.
- Provide symptom management and palliative support to patients and families.
- Complete history & physical assessments and clinical documentation.
- Collaborate with physicians, nurses, social workers, and the interdisciplinary hospice team.
- Communicate clinical findings and recommendations to support quality patient care.
What We're Looking For:
- Active Texas Nurse Practitioner license.
- PECOS enrollment required for hospice certification requirements (or willingness to obtain).
- Minimum 1 year of NP experience required.
- Hospice, palliative care, family practice, home health, internal medicine, or related experience preferred.
- Strong assessment, communication, and critical thinking skills.
- Ability to work independently while collaborating with a team.
