    As a Nurse Practitioner specializing in in-home and hospice care, you’ll have the flexibility to manage your schedule while delivering personalized, dignified care to patients and families during life’s most important moments.

    • Conduct in-home visits, physical assessments, and face-to-face evaluations
    • Develop and manage individualized care plans in collaboration with physicians and care teams
    • Prescribe medications and treatments, ensuring symptom control and comfort
    • Educate and support patients and families through end-of-life care decisions
    • Serve as a trusted resource for hospice eligibility and recertification

    Requirements

    • Master’s degree and completion of an accredited Nurse Practitioner program
    • Current state licensure and CPR certification
    • Certification in Hospice and Palliative Nursing (CHPN) or specialty areas such as geriatrics or pediatrics preferred, but not required
    • At least 1 year of NP experience and 1 year in hospice or palliative care
    • Strong communication, problem-solving, and clinical skills
    • Valid driver’s license, auto insurance, and reliable transportation
