    Position Overview/Purpose:

    We are looking for an experienced Per Diem Nurse Practitioner to provide face-to-face assessments care to hospice patients in the Wichita, KS region. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases.

    Principal Duties and Responsibilities (Essential Functions):

    • Review daily schedule and visit locations

    • Confirm all required Face-to-Face (F2F) encounters due within regulatory timeframe

    • Review patient hospice eligibility, terminal diagnosis, and prior certifications

    • Review recent:

    • Nursing notes

    • IDG notes

    • Hospitalizations/ER visits

    • Medication changes

    • Decline indicators

    Before Each Visit

    • Review patient history and prior F2F documentation

    • Identify:

    • LCD eligibility indicators

    • Functional decline

    • Weight loss/nutritional decline

    • PPS/FAST trends

    • Cognitive changes

    • Symptom burden

    • Confirm caregiver/contact availability

    • Notify family/facility of estimated arrival if needed

    Eligibility Documentation

    • Clearly document continued decline supporting hospice eligibility
    • Include objective findings:
    • PPS score
    • FAST score (if dementia)
    • MAC/weight loss
    • Frequent infections/hospitalizations
    • Increased dependence
    • Ensure documentation supports physician recertification
    • Avoid vague wording such as "stable" without context

    Medication Review

    • Reconcile medications
    • Review comfort medications and effectiveness
    • Identify unnecessary or non-covered medications
    • Address symptom management concerns
    • Communicate recommendations to hospice physician/IDT as needed

    Communication & Coordination

    • Educate patient/family on disease progression and expectations
    • Reinforce goals of care and hospice philosophy
    • Collaborate with:
    • Case manager
    • Medical director
    • Escalate urgent concerns immediately
    • Update clinical leadership regarding high-risk patients

    Documentation Checklist

    • Complete F2F note same day
    • Ensure note includes:
    • Reason for encounter
    • Clinical findings
    • Decline indicators
    • Functional status
    • Prognosis indicators
    • Eligibility statement
    • Sign documentation promptly

    Qualifications/Skills:

    • Valid certification in state of job location
    • Strong people and rapport building skills with positive and personable demeanor
    • Demonstrated collaboration skills
    • Excellent communication skills
    • strong and confident presentation skills
    • 1-2 years of primary care experience with older adults

    Salary Range:

    • $85 per visit + (depending on experience and other incentives)
