We're building our PRN Hospice Nurse Practitioner team and offering competitive per-visit pay with the flexibility to work on your terms.Pay Rates – Per Completed VisitPer Visit - $90Downtime (pre-approved) - $60/hour"Completed" = visit done in the home + documentation finished within 24 hours + orders/signatures uploaded + meets QA standards.Service Area & ScheduleCleveland / Northeast OhioWhat You'll DoConduct face-to-face hospice eligibility visits and complete required documentation in compliance with Medicare hospice regulationsPerform comprehensive assessments of hospice patients in their homes, including symptom management evaluation and goals-of-care discussionsCollaborate with the attending physician and interdisciplinary hospice team to develop and adjust individualized plans of carePrescribe and manage medications focused on comfort, pain management, and symptom controlProvide advanced clinical guidance to hospice field staff including RNs, LPNs, and aidesEducate patients and families on disease progression, comfort-focused care, and end-of-life planningParticipate in interdisciplinary team meetings and case conferences as neededMaintain accurate and timely documentation in the EMR within required timeframesEnsure compliance with Medicare, Medicaid, and CHAP hospice standardsWhat Sets VNA Apart4-star CMS agency with CHAP accreditation and a 124-year legacy in Northeast OhioMileage reimbursement at the IRS standard rate ($0.70/mile)Paid training & CEUs through ReliasLimited weekend/holiday requirements - we respect your time offResponsive support team - scheduling, intake, clinical supervisors, triage, and after-hours coverage so you're never on an islandRequirementsMust-Have:Active NP license (Ohio) + current BLS certificationMaster's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP)Active DEA registrationReliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service areaMinimum 2 years of advanced practice nursing experienceFamiliarity with Medicare and Medicaid hospice regulationsPreferred / Nice-to-Have:Prior hospice or palliative care NP experienceACHPN certification or willingness to pursueExperience conducting face-to-face hospice eligibility visitsFamiliarity with HCHB or similar home health/hospice EMRStrong goals-of-care conversation and family communication skills
