Job Duties:
Coordinates hospice care by workingwith other team members as well as with the patients/family members to ensureneeds are met.
Evaluate the effectiveness ofhospice services to the patient and family on an ongoing basis.
Perform evaluations for hospice carealong with informational visits as needed.
Utilize the nursing process toprovide professional nursing care.
Submit patient visit scheduleweekly.
Provide guidance and supervision tothe LPN and supervises the LPN a minimum of every 30 days.
Prepare the care plan for theHospice Aide/Home Attendant.
Requirements
Must be a graduate from an accredited School of Nursing
Must be licensed in the State of Virginia as a RegisteredNurse
Hospice/Home care experience preferred (extensive training provided)
