Hospice Registered Nurse Case Manager

Title: Hospice RN Case Manager

Location: Milwaukee Wisconsin 53213

Duration: 13 Weeks

Shift: 8hr Day Shift (40)

Shift Information: Monday through Friday 0800 to 1630

On Call, Call Back Requirements: No oncall hours

Weekend Rotation: No weekends

Holiday Requirements: No holiday

Floating Requirements: May have to float to surrounding teams if needed.



About Facility:

• Home Care remote field work. Locations include Ozaukee and Washington counties.

General Information:

• Beds: N/A
• Will you accept a traveler with 1-year experience: Yes
• Will you accept a first-time traveler: Yes, as long as they have 1 years of Home Health experience.

Patient Types:

• Staff will primarily be seeing patients in the community setting, i.e. their home, the home of a family member, Assisted Living Facilities, Group Homes, Nursing Homes
• High acuity patients: pleurx drains, PCA pumps, IV ABX, TPN, Foleys, wounds, wound VAC, chest tubes, various drains

Patient Ratios:

• Remote/Field:
• RN Case Manager: Caseload is 22-25 a week; 6 visits a day – if the traveler is unable to keep up with the caseload, please let leadership know

Required Certifications:  BLS

Skills required:

• Patient/family education
• Provider communication
• Disease management
• Troubleshoot/knowledge of pleurx drains, nephrostomy tubes, trachs, PCA s, gastric suctioning; will not perform these tasks, just knowledge on troubleshooting – will be trained on site if needed

Support on the Unit:

• Case Managers
• Providers
• Clinical Manager
• Chaplain
• Social Workers

Technology/Equipment:  EMR: EPIC – experience preferred

Floating: varies

Orientation (how many shifts): Up to 2 weeks, based on comfort level

Scheduling:

• Weekend rotation: no weekends
• Call: No call
• Holiday Expectations: no holidays
• Approval of time off: Manager to approve RTO
• Shift times: Shift times: M-F 0800-1700 (must be available by phone during these hours); Daily huddle at 07:50 am, can be attended by phone or teams if the traveler is on the way to see patients. Flexible schedule M-F or Tues-Sat, accepted. We are looking for a traveler open to working Saturdays if needed.

• Laptop, cell phone, and PPE will be provided
• Must be N95 fit tested prior to arrival
