Due to growth we are hiring RN's with at least 1 year of experience for Hospice Clinical Coordinator Roles, Call Center Triage, Field Case Manager, Admissions.

Job Details

Depending on the opening, responsibilities may include:
• Conduct hospice admissions, assessments, eligibility evaluations, and care planning for patients entering hospice services.
• Manage a caseload of hospice patients, coordinating care plans, symptom management, family education, and interdisciplinary team collaboration.
• Provide inpatient hospice support within hospital settings, partnering with physicians, case managers, and clinical teams to facilitate transitions of care.
• Deliver triage support, responding to patient and family concerns, symptom management needs, and urgent clinical issues during assigned shifts.

• Hospice RN Case Manager – South Brooklyn (Mandarin or Cantonese preferred)
• Inpatient Clinical Care Coordinator Hospice RN – NY Presbyterian (multiple openings)
• Hospice RN Case Manager – Bronx (Spanish speaking)
• Hospice Triage RN – Bronx (Evening and Overnight shifts)

Skills You Need
• Active RN license with experience in Hospice, Palliative Care, Oncology, Hospital, Rehabilitation, Skilled Nursing, Home Health, or other settings supporting hospice patients.
• Strong clinical assessment, care coordination, communication, and end-of-life care skills. Hospice, Palliative Care, Oncology, ICU, or PM&R experience highly valued.
