Optimal Care is where your dedication meets a rewarding career.

Top Work Places for 12 consecutive years, Great Places to Work certified for 6 consecutive years, and we believe that exceptional care starts with exceptional people. We're committed to supporting your professional growth, valuing your expertise, and creating an environment where you can do your best work every day.

As a clinician-owned and operated organization, we empower our team members to provide personalized, compassionate care to patients and families every day.

We are a leading provider of Physician Services, Home Health, and Hospice that encourages collaboration, innovation, and clinical excellence. From chronic disease management and rehabilitation to end-of-life care, our focus is on improving outcomes and enhancing quality of life.

We live a simple Mission: Serve Together, Provide Value, and Deliver Exceptional Quality Care.

About the Role

Combine compassionate hospice nursing with the opportunity to develop the next generation of clinicians. As a Hospice Nurse Preceptor, you'll provide skilled end-of-life care to patients and families while serving as a trusted mentor, educator, and clinical resource for nurses throughout their onboarding and professional development. Your expertise will help ensure both exceptional patient care and the continued growth of a confident, highly skilled clinical team.

This role is ideal for an experienced Hospice RN who is passionate about sharing knowledge, leading by example, and fostering clinical excellence. You'll manage a patient caseload, coordinating individualized care from admission through ongoing visits while building meaningful relationships with patients, families, physicians, and the interdisciplinary care team. At the same time, you'll guide new clinicians through orientation, provide ongoing coaching and remedial education when needed, and help students and staff build the confidence and competencies required to deliver outstanding hospice care.

Working closely with clinical leadership and the Education Department, you'll help promote best practices, reinforce quality standards, and support a culture of continuous learning. Through your clinical expertise, mentorship, and compassionate leadership, you'll directly influence both patient outcomes and the success of the clinicians who care for them.
