We are partnering with a well-established, multi-state hospice provider known for delivering compassionate, patient-centered care through interdisciplinary teams in both home and facility settings. We're seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position (40 hours per week).
Pay and Schedule
$40–$46/hour
Monday–Friday, 8:00am–5:00pm
On-call and weekend rotation approximately every 13 weeks
Includes weekend coverage (Saturday and Sunday)
Includes holiday participation (minimum one summer and one winter holiday)
$6/hour hold rate plus time-and-a-half for visits during call

60% of visits are facility-based, supporting more efficient scheduling

Responsibilities
Manage hospice patient caseload across home and facility settings
Perform patient assessments and develop individualized care plans
Provide skilled nursing care and symptom management
Coordinate with interdisciplinary team members
Maintain documentation using Homecare Homebase
Participate in on-call rotation
Territory
Coverage includes Dauphin, Lebanon, Cumberland, Perry, Franklin, and Adams counties
Patients are assigned geographically to support efficiency
Typical day includes approximately one hour of drive time
Mix of facility and home-based hospice care

Active RN license
