Job SummaryWe are seeking a
HospiceRegistered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
- Strong Hospice Nursing experience required
- Strong Case Management experience preferred
- Previous Hospice On-Call experience required
- Hospice Admissions experience required
- Experience with Axxess EMR required (will train)
Responsibilities
Completes initial, comprehensive, and ongoing assessments of the patient and family to determine hospice needs.
Conducts a complete physical assessment and obtains a detailed history of current and previous illnesses.
Provides professional nursing care utilizing all elements of the nursing process.
Assesses and evaluates the patient’s status by:
- Developing and initiating the plan of care
- Regularly re-evaluating patient and family/caregiver needs
- Participating in revisions to the plan of care as necessary
- Using health assessment data to determine nursing diagnoses
Develops an individualized care plan that establishes goals based on nursing diagnoses and incorporates palliative nursing interventions. Includes the patient and family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures.
Administers medications and treatments as prescribed by the physician and outlined in the plan of care.
Counsels the patient and family regarding nursing and related needs.
Provides health care instructions based on assessment findings and the established plan of care.
Assists the patient with activities of daily living and supports efforts toward self-sufficiency and optimal comfort.
Functions as Case Manager, assuming responsibility for coordinating patient care for the assigned caseload.
Completes, maintains, and submits accurate and relevant clinical documentation regarding the patient’s condition and care provided. Records pain and symptom management changes and outcomes as appropriate. Communicates with the physician regarding patient needs, reports changes in condition, and obtains physician orders as required. Coordinates care with community health professionals as needed.
Educates the patient and family/caregiver in self-care techniques. Provides medication, diet, and other instructions as ordered by the physician. Utilizes opportunities for health counseling.
Collaborates with the interdisciplinary group to ensure continuity of care. Maintains a safe environment for the patient. Works with the family/caregiver and hospice team members to meet the emotional needs of the patient and family.
Attends interdisciplinary group meetings.
Additional Duties:
- Participates in on-call responsibilities as outlined in the on-call policy
- Ensures necessary equipment, supplies, and services are arranged
- Supervises ancillary personnel and delegates responsibilities appropriately
- Engages in personal professional development through continuing education and in-service training
- Fulfills requested and accepted case assignments
- Actively participates in quality assessment and performance improvement activities
Qualifications- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse, working with Hospice patients/case load is preferred
- Excellent interpersonal and client care skills
- Valid driver’s license and Car Insurance
- First aid/CPR certification required
