About the Role

Our client is seeking an experienced and compassionate Registered Nurse (RN) to join a highly respected hospice team serving patients throughout the Green Bay area.

In this role, you will provide comprehensive hospice nursing care while serving as the primary clinical case manager for an assigned patient population. You'll collaborate closely with physicians, Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), social workers, chaplains, and other members of the interdisciplinary care team to ensure every patient receives exceptional, individualized end-of-life care.

This position offers an outstanding work-life balance with a Monday through Friday schedule (8:00 AM – 5:00 PM), no routine call responsibilities, and an average caseload of 5–6 patient visits per day within approximately a 60-mile radius of the branch office.

What You'll Do

• Serve as the primary case manager for assigned hospice patients.

• Perform comprehensive initial and ongoing nursing assessments.

• Develop, implement, and monitor individualized Plans of Care in collaboration with physicians and the interdisciplinary team.

• Deliver compassionate nursing care focused on symptom management, comfort, and quality of life.

• Educate patients, families, and caregivers regarding hospice philosophy, medications, treatments, and disease progression.

• Coordinate care among physicians, nurses, aides, therapists, and other healthcare professionals.

• Document all patient care accurately and maintain complete electronic medical records.

• Monitor changes in patient condition and communicate appropriately with attending and hospice physicians.

• Participate in Quality Assessment and Performance Improvement (QAPI) initiatives.

• Supervise and mentor assigned clinical staff while promoting best nursing practices.

• Conduct routine supervisory visits with team members.

• Ensure compliance with Medicare, Medicaid, accreditation, and agency policies.

• Attend interdisciplinary meetings and staff education sessions.

• Maintain infection prevention standards and patient safety practices.

Schedule

• Monday – Friday

• 8:00 AM – 5:00 PM

• No routine call

• Average of 5–6 patient visits per day

• Approximately 60-mile travel radius from the Green Bay branch

Required Qualifications

• Current Registered Nurse (RN) license in the State of Wisconsin (or ability to obtain prior to start).

• Minimum of one year of RN experience in one or more of the following:

  • Hospice

  • Home Health

  • Acute Care

  • Oncology

  • Long-Term Care

• Clinical assessment and case management experience.

• Strong knowledge of hospice philosophy and end-of-life patient care.

• Excellent communication and interpersonal skills.

• Ability to effectively coordinate care across multiple disciplines.

• Experience maintaining accurate electronic clinical documentation.

• Working knowledge of Medicare, Medicaid, and regulatory compliance standards.

• Current American Heart Association (AHA) Basic Life Support (BLS) certification.

• Valid driver's license, reliable transportation, and current automobile insurance.

• Ability to travel throughout the assigned service territory.

Preferred Qualifications

• Experience using Homecare Homebase (HCHB) electronic medical record system (Required for this assignment).

• Previous hospice case management experience.

• Experience leading interdisciplinary care coordination.
