About the Role

Our client is seeking an experienced Hospice Registered Nurse (RN) to provide compassionate, patient-centered hospice care throughout the Monroe, North Carolina service area. This position offers an excellent weekday schedule with manageable daily patient volumes, allowing clinicians to focus on delivering exceptional end-of-life care while maintaining a healthy work-life balance.

The Registered Nurse serves as the primary clinical case manager, coordinating care for hospice patients while collaborating with physicians, Licensed Practical Nurses, Certified Nursing Assistants, social workers, chaplains, and other interdisciplinary team members. The ideal candidate is organized, clinically confident, and passionate about providing dignity, comfort, and support to patients and their families during every stage of the hospice journey.

Key Responsibilities

• Manage an assigned caseload of hospice patients while providing comprehensive nursing care.

• Perform admission, routine, and follow-up patient visits.

• Conduct initial and ongoing patient assessments.

• Develop, implement, and update individualized Plans of Care in collaboration with physicians and the interdisciplinary team.

• Coordinate patient services to ensure high-quality, compassionate hospice care.

• Educate patients, families, and caregivers regarding hospice philosophy, medications, symptom management, oxygen therapy, dressing changes, and disease progression.

• Monitor patient condition and promptly communicate significant clinical changes to the attending physician and hospice physician.

• Maintain accurate, timely, and compliant electronic medical documentation.

• Review patient clinical records to ensure quality, regulatory compliance, and adherence to clinical standards.

• Perform supervisory visits with nursing staff and other care team members as required.

• Participate in Quality Assessment and Performance Improvement (QAPI) initiatives.

• Practice infection prevention and standard safety precautions.

• Attend staff meetings and complete required compliance education.

• Promote a collaborative, patient-focused culture while supporting agency core values.

Required Skills

• Active Registered Nurse (RN) license

• Previous hospice nursing experience

• Homecare Homebase (HCHB) experience required

• Strong clinical assessment and care planning skills

• Hospice case management experience

• Excellent documentation skills

• Electronic Medical Record (EMR) proficiency

• Strong communication and interpersonal abilities

• Ability to work independently in the field

• Compassionate, patient-centered approach to end-of-life care

• Valid driver's license and reliable transportation

• Ability to travel within a 60-mile service radius

Preferred / Nice-to-Have Skills

• Previous home health experience

• Experience working within an interdisciplinary hospice team

• Quality improvement or compliance experience

• Strong patient and family education skills

• Supervisory or leadership experience
