In this full-time Hospice Registered Nurse role based in Wexford, PA, you will be integrated into an exceptional care team. Required Qualifications:
- An associate degree or higher
- A minimum of one year's experience with TruChart EHR, EMR, or other EMR platforms
- Valid RN certification or license within the state of Pennsylvania
- Proficiency in hospice care, with at least two years of applicable experience
Key Responsibilities:
- Collaborative development and implementation of patient care plans
- Provide support to patients within Wexford, PA and neighboring zones
- Efficient time management for meeting critical charting deadlines
- Proficiency with the use of tablets, mobile phones, and EMR software
Schedule and Shift Details:
40 hours per week, daytime shifts, with occasional on-call duties. Local travel is a requirement.
