Vantage Hospice is expanding into the Bryan/College Station area! We are growing and seeking an experienced, compassionate RN Case Manager to join our team.
Position Reports To: Clinical Manager

Qualifications
Current licensure/registration as a Registered Nurse (RN) in the State of Texas.
Previous hospice nursing experience preferred.
Experience caring for terminally ill patients and families with strong knowledge of symptom control and emotional support.
Background in Oncology, Home Health, or Long-Term Care is a plus.
Excellent physical and psychosocial assessment skills with strong reasoning, deduction, and critical thinking abilities. Must demonstrate independence in clinical assessment.
Ability to prioritize urgent patient, family, and facility needs while managing multiple tasks simultaneously.
Strong time-management and organizational skills with flexibility in scheduling.
Must display a professional attitude and demeanor at all times.
Reliable transportation, valid Texas driver’s license, and state-required liability insurance (or access to reliable transportation) required.

Responsibilities
Conduct comprehensive patient assessments and develop individualized hospice care plans.
Coordinate and manage patient care services while ensuring compliance with HIPAA and all regulatory guidelines.
Collaborate with the interdisciplinary team to support discharge planning and post-acute care needs.
Educate and support patients and families regarding hospice services and end-of-life care.
Maintain accurate, timely documentation of patient progress and care interventions in accordance with agency policies.
