Hospice RN Case Manager

The Hospice RN Case Manager plays a vital role within a multidisciplinary team by providing expert nursing care and support to hospice patients and their families. This position focuses on comprehensive patient assessments, care planning, and coordination to ensure compassionate end-of-life care, symptom management, and adherence to compliance standards.

Responsibilities

• Conduct thorough patient assessments to determine care needs
• Develop and implement individualized care plans
• Manage medications ensuring proper administration and monitoring
• Monitor patient symptoms and adjust care as necessary
• Provide counseling and emotional support to patients
• Support family members through education and guidance
• Coordinate care among healthcare providers and services
• Maintain accurate and comprehensive documentation
• Collaborate with interdisciplinary team members for holistic care
• Ensure compliance with hospice care regulations and standards
• Advocate for patients to uphold their rights and preferences

Required Qualifications

• Associate Degree in Nursing (ADN)

Preferred Qualifications

• 1+ years of experience in hospice nursing
