Hospice RN Case Manager - Field / Visiting Nurse

***NOTE: THE PAY IS $55-100 PER VISIT, depending on the type of visit. Plus mileage. IT'S NOT PER HOUR**

Local Hospice Company is looking for a compassionate and dedicated Hospice RN Case Manager to join our field-based team in Saint Charles, IL. Responsibilities:

• Manage a caseload of hospice patients, conducting in-home assessments and developing individualized care plans
• Perform skilled nursing visits to monitor patient symptoms, pain management, and overall comfort
• Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and aides to coordinate holistic patient care
• Educate and support patients and their families on disease progression, medication management, and comfort care techniques
• Document clinical findings, care plans, and visit notes accurately and in compliance with hospice regulations
• Ensure compliance with Medicare/Medicaid hospice guidelines and agency policies
•  Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 
•  Participates in on-call duties as defined by the on-call policy. 
•  Ensures that arrangements for equipment and other necessary items and services are available. 
•  Attends bi-monthly interdisciplinary group meetings. 

Requirements:

• Active RN license in the state of Illinois (required)
• Minimum 1-2 years of clinical nursing experience; hospice, palliative care, or home health experience preferred
• Strong knowledge of pain and symptom management in end-of-life care
•  Maintains a current CPR certification. 
• Excellent communication and interpersonal skills with patients, families, and care teams
• Ability to work independently in a field-based, self-directed environment
• Valid driver's license and reliable transportation for home visits in the Saint Charles, IL area
• Proficiency with electronic medical records (EMR) documentation
• Empathetic, patient-centered demeanor with a commitment to dignity and compassionate care
