As a Hospice RN Case Manager, you'll be a trusted source of comfort, support, and guidance for patients and their families during one of life's most important transitions. You'll provide compassionate, patient-centered care while helping families navigate end-of-life care with dignity.

Responsibilities

• Manage and coordinate hospice care for an assigned patient caseload
• Complete comprehensive nursing assessments and monitor changes in patient condition
• Develop, implement, and update individualized plans of care
• Provide education, emotional support, and guidance to patients and their families
• Collaborate closely with physicians and the interdisciplinary hospice team
• Maintain accurate, timely, and thorough clinical documentation
• Participate in the on-call rotation as assigned
• Advocate for patients and ensure care is delivered with compassion, dignity, and respect

Qualifications

• Graduate of an accredited school of nursing
• Current, active RN license in Kansas and Missouri, a compact license, or the ability to obtain the required license(s) within 30 days of hire
• RN license(s) must be in good standing
• Minimum of 1 year (12 months) of recent Registered Nurse experience
• Hospice, home health, palliative care, oncology, or medical-surgical experience preferred
• Valid driver's license, reliable transportation, and current auto insurance
• Strong assessment, communication, time-management, and organizational skills
• Compassionate, patient-focused approach to end-of-life care
