Hospice RN Case Manager

The Hospice RN Case Manager plays a vital role in delivering compassionate, comprehensive care to patients and their families during end-of-life stages. This position requires skills in patient assessment, symptom and pain management, and coordination across interdisciplinary teams to optimize patient comfort and quality of life.

Responsibilities

• Conduct thorough patient assessments to evaluate health status and care needs
• Develop and implement individualized care plans in collaboration with healthcare team
• Manage symptoms and provide effective pain control to enhance patient comfort
• Offer emotional support and education to patients and family members
• Coordinate interdisciplinary care among nurses, physicians, social workers, and other staff
• Maintain accurate, complete documentation in compliance with healthcare regulations
• Monitor adherence to hospice care standards and policies
• Advocate for patient rights and preferences throughout hospice care
• Provide education and training related to hospice care best practices

Preferred Qualifications

• 2+ years of experience in hospice nursing case management
• Associate Degree in Nursing (ADN)
• Proficiency in patient assessment, care coordination, pain and symptom management
• Experience with Electronic Medical Records (EMR) systems
• Strong communication, empathy, and time management skills
• Ability to collaborate effectively within interdisciplinary teams
