Location: Cherry Hill NJSchedule: Monday–Friday | 8:00 AM–5:00 PMOn-Call: Rotating | Limited occasional evenings/weekendsCaseload: Approximately 15 patients (subject to change based on census and acuity)

Your Role

As an RN Case Manager, you are the clinical lead for your assigned patients — supported by a strong interdisciplinary team.

You will:

• Conduct comprehensive nursing assessments (with Admissions Nurse support)
• Develop and manage individualized plans of care
• Collaborate with physicians, LPNs, social workers, chaplains & aides
• Educate families on symptom management and disease progression
• Participate in IDT meetings
• Maintain timely, compliant documentation

• Qualifications

• Active New Jersey RN License
• Hospice or home health experience strongly preferred
• Strong clinical judgment & communication skills
• Valid driver's license & reliable transportation
