Job Details

Seeking a Hospice RN Case Manager / Registered Nurse Hospice to manage a structured, low-acuity caseload across a clustered Upper Manhattan territory.

This role is ideal for an RN looking to transition into a more balanced, organized hospice setting with strong support and realistic expectations.

What You’ll Do:

Manage a caseload of ~4 patients per day (home, SNF, inpatient settings)
Develop and adjust care plans focused on comfort and quality of life
Coordinate with LPNs, HHAs, and interdisciplinary care teams
Educate patients and families on care plans and symptom management
Conduct assessments and ensure proper documentation

Schedule:

Monday–Friday, 8:30am–5pm
1 weekend per month

This is a true case manager RN role — not high-volume field chaos — with a focus on quality care, support, and sustainability.
