We have created a new team for Midtown Manhattan and are seeking RN's who have a passion for hospice care. Hospice experience, which could include nursing home experience, is required.

Job Details

RN Case Manager seeking on average only 4 patients per day within facilities a homes. Roughly 75% facilities.


What You'll Be Doing
• Manage a caseload of hospice patients throughout Midtown Manhattan, completing routine visits in both skilled nursing facilities and patient homes (approximately 75% facility-based care).
• Perform comprehensive assessments, develop individualized care plans, and provide symptom management focused on comfort and quality of life.
• Educate patients, families, and caregivers regarding disease progression, medications, hospice services, and end-of-life care.
• Collaborate with physicians, social workers, chaplains, and the interdisciplinary care team to coordinate and deliver exceptional hospice care.

Skills You Need
• Active RN license with experience in Hospice, Palliative Care, Oncology, Hospital, Rehabilitation, Skilled Nursing, Home Health, or another setting where you've worked directly with hospice or end-of-life patients.
• Strong clinical assessment, care coordination, communication, and patient/family education skills with a compassionate, relationship-focused approach to care.
