A bit about us:

We are working with a long-established nonprofit healthcare organization providing hospice and palliative care to patients and families throughout New York City.

There are opportunities across multiple boroughs, and candidates can be considered for the opening and territory that best align with where they live. You will not be handed an entire borough and told to figure it out. Each nurse works within a smaller, concentrated territory designed to make the daily schedule more manageable.

Most nurses complete approximately four patient visits per day, creating more time to assess patients properly, support families and coordinate care without turning every visit into a race against the clock.

Transportation needs vary by assignment. Some territories require driving, while others can be covered using public transportation. A car is not required for every position, so candidates should not automatically rule themselves out based on transportation.

Job Details

WHAT YOU WILL BE DOING
Complete physical and psychosocial assessments for patients with end-stage illnesses
Develop, implement and update individualized hospice nursing plans of care
Provide symptom management, education and emotional support to patients and families
Coordinate changes in care with physicians and the interdisciplinary hospice team
Supervise, train and evaluate home health aides and personal-care workers assigned to patients
Complete accurate clinical documentation and participate in scheduled backup on-call coverage

SKILLS YOU NEED
Required: Active New York State Registered Nurse license
Required: Associate degree or nursing diploma from an accredited nursing program; BSN preferred
Preferred: At least two years of medical-surgical, community health, oncology, home health or hospice nursing experience
Transportation: Ability to travel throughout the assigned territory; a valid driver’s license and vehicle are required only for openings with driving-based territories
