Position Summary
The Hospice RN Case Manager provides skilled nursing care to hospice patients while coordinating services, managing symptoms, supporting families, and collaborating with the interdisciplinary team. This role is responsible for helping ensure every patient receives individualized, high-quality, compassionate care focused on comfort, dignity, and quality of life.
Responsibilities:
- Complete admissions, nursing assessments, and re-certifications to determine hospice eligibility and level of care.
- Assess physical, functional, psychosocial, environmental, safety, and ADL needs.
- Evaluate pain and symptoms and develop individualized pain management plans.
- Assess caregiver ability to meet immediate and ongoing patient needs.
- Develop and implement individualized plans of care focused on comfort, symptom relief, and quality of life.
- Determine visit frequency and service needs based on patient acuity and family needs.
- Provide skilled nursing care using appropriate preventative and rehabilitative interventions.
- Coordinate patient and family services using a case management approach.
- Collaborate with physicians and the interdisciplinary team to ensure continuity of care.
- Educate patients, families, and caregivers on disease progression, self-care, end-of-life care, and ethical concerns.
- Document assessments, care plans, interventions, and outcomes accurately and timely.
- Attend interdisciplinary group meetings and participate in care plan updates.
- Support the patient’s and family’s spiritual, cultural, and personal beliefs.
- Provide holistic, family-centered care across care settings.
- Train family members, caregivers, and staff on hospice-related care needs as appropriate.
- Participate in on-call rotation as requested.
- Complete required documentation, re-certification, discharge, and payroll paperwork on time.
- Participate in orientation, in-service education, and performance improvement activities.
- Maintain professionalism, reliability, accountability, and strong communication in all settings.
- Follow all agency policies, procedures, and standards of care.
- Perform other duties as assigned.
Qualifications:
- Current, active Arizona RN license.
- Must meet Arizona Board of Nursing requirements, including fingerprint clearance requirements.
- Current CPR certification.
- Health clearance and clinical skills self-assessment.
- Minimum 1 year of professional nursing experience.
- BSN preferred, or qualifying nursing experience within the last 3 years.
- Hospice, home health, palliative care, or case management experience preferred.
- Strong assessment, documentation, communication, and time-management skills.
- Ability to work independently while collaborating closely with an interdisciplinary team.
